At SilverStone Hospice We Are Called To Care

At SilverStone Hospice, we believe we were called for something greater – we are called to care.

Fiercely compassionate, we vigilantly support our patients in their final hours, so no one dies alone.

Every patient is a person with hopes, dreams, and dignity. Every day, every patient in our care deserves someone to be present in the moment providing a higher level of end-of-life care. Care that comes from the heart.

It’s why we only hire compassionate, proactive and attentive care providers. People who provide the highest quality healthcare through selfless acts of service and companionship in every life we touch. We are on a mission to transform hospice care and are guided by ethics and values grounded in the respect for every need, wish and aspiration of our patients and families.

We provide quick responses, clear expectations and communication around the “whys” and “hows” for any circumstance. Our services are transparent, medically educated, and process driven.

We’re answering more than just a call for healthcare. We’re answering a call to care.

Inspiration Through Service

Built atop a decade-old hospice organization, Silverstone was founded by Alfonso Montiel, a successful entrepreneur and vigil volunteer for hospice patients. Alfonso saw an opportunity for higher quality care within the hospice industry and set out to change the way people view hospice.

Inspired by the mission-led works of Dame Cicely Saunders, the renowned nurse, social worker, and physician who created modern-day hospice and palliative care, Alfonso set out to deliver hospice services through its original vision of incorporating the four pillars of care: physically, emotionally, spiritually, and socially. Hospice care that rewards the individual’s life.

"You matter becasue you are you, you matter to the last moment of your life."

~ Dame Cicely Saunders

Always Present

SilverStone was created to honor our patients’ lives. With generous spirits and servant’s hearts, our team provides the gift of being present for our patients and their families.

Every day we help support the loving efforts of families and caregivers to help patients receive a higher quality of life and the care they deserve. To us, hospice doesn’t mean giving up, it means giving each a voice to the hope and desire of each person we serve.

Our patient and family-centered care improve and enhance the quality of life by anticipating, preventing, and treating symptoms to bring comfort. Our Board-Certified Medical Directors facilitate a caring environment and lead our team of nurses, aides, spiritual counselors, social workers, and volunteers to provide comprehensive treatment tailored to our patient’s unique needs

No One Dies Alone Program

With Us, No One Dies Alone. We are present before, during and after a patient’s transition.

Our No One Dies Alone Program brings specialized team members skilled in transitional care to support our patients and families as they are in their final 24-48 hours.

Our communities will have the best care and support before, during and after death. We are fiercely compassionate as we vigilantly support our patients in their final hours so that no one dies alone.

We sit bedside and hold the hand of our patients as we read poems and listen to their favorite music. We master the art of being present and wait by their side as our patient’s transition. Our vigil team is always accessible to our loved ones and families, even in the most challenging circumstances.

"In hospice, we don’t get do-overs; we have to get things right every single time, for every single family; and this is how we do things at SilverStone Hospice."

~ Gloria Flack, MSN, RN, SilverStone Administrator

Patients and Families Come First

We empower families. By mirroring the family system, we naturally become an authentic extension of care, love and support. We enable families to confidently provide the best care experience for their loved ones.

What Makes SilverStone Hospice Unique?

Volunteering, the foundation of how our company originated is the exact same as what started hospice in the United States 50 years ago; our CEO was a vigil volunteer for hospice patients, volunteering became his calling and he bought the company.

We create tried and true, individualized plans of care that reinforce every patient support system, and we go out of our way to advocate on our patients’ behalf.

We operate with transparency, honor, integrity, and doing the right thing because it is the right thing to do.

We hire a very specific type of person to join SilverStone: Clinically experienced and qualified, consistently provides care filled with compassion, strives to say “yes” to our patients’ needs, to meet them where they are in their journeys, and sees hospice more than just a career, but as a calling.

We provide palliative care versus curative care as we aggressively treat symptoms. In hospice we don’t get do-overs; we have to get things right every single time, for every single family; and this is how we do things at SilverStone.
