What is the Transitional Care Program?

SilverStone Hospice, in partnership with our home health agency, is able to provide effective transitional care models for acute patients discharging from facilities or currently residing at home. Over the last year, we have been able to reduce unnecessary hospital readmission while providing personalized care in the home setting.

Services

  • Care is initiated within 24 hours of discharge.
  • Additional Skilled Nursing visits.
  • Appointments scheduled with primary care physician or visiting physician within 14 days of discharge.
  • Remote monitoring for CHF and COPD patients.
  • Social Services for advance directive preparation.

Qualification:

You may qualify for this program if you are diagnosed with a life limiting illness that requires regular physician visits, on-going unmanaged symptoms or medication management.

As part of our Transitional Care Program, we partner with VCare Home Health to provide services to fulfill your daily needs. As part of our mission to provide a spectrum of care yielding pristine environments, SilverStone Hospice in partnership with VCare Home Health provides Home Health services to the Dallas, Denton and Collin counties in Texas.

Transitioning care is crucial to patients with complex needs who are moving between different levels of care. For our patients and families who may not be ready for palliative or end-of-life care, we offer a skilled care program to provide additional care for your loved one in the comfort of their own home.

A team of trained nurses, physical therapists, occupational therapists, speech therapists, and social workers, supervised by your primary care physician or our visiting physician, will provide a variety of skilled care services to ensure that your loved one’s needs are being met.
