Our team of Transitional Care Consultants will coordinate every step of your loved one’s transport to the comfort of your home.

Our team provides:

  • To both prevent and manage COVID-19 related risks and symptoms, a specialized COVID-19 Transitional Care Team including a nurse, aide and volunteer dedicated specifically to the care of you and your loved one.
  • Full PPE (Personal Protective Equipment) including masks, gloves, gowns and goggles.
  • A COVID-19 Care Kit for you and your family.

Our care team is prepared with N95 masks, gowns, gloves and additional PPE for the facility care team and staff of our patients. We provide virtual Face-Time visits with family members of patients who are cared for in a skilled nursing facility each week. SilverStone Hospice staff is tested every 14 days for Covid -19. 

In the midst of the COVID-19 pandemic, our Seamstresses with Halo’s volunteer team have a weekly production of masks for nurses and care team members in partner facilities and hospitals.

During the COVID-19 crisis and wide-spread shut down of Skilled Nursing Facilities, our No One Dies Alone volunteer team provided care for patients within a facility by passing the baton of compassionate care and sitting at the bedside of our patients when the family is unable to be present. With SilverStone Hospice, no one dies alone. We are fiercely compassionate and vigilantly support our patients in their final hours, so that no one dies alone.

We will continue to review not only how we operate now, but also how we will operate over the coming weeks and months. We are prepared to support as many patients, families and caregivers as possible, either directly or indirectly.

Dear Colleague,

As the new administrator for SilverStone Hospice, I’m reaching out to you to confirm both our preparedness and readiness to support your team during this time of uncertainty resulting from the impact of COVID-19. SilverStone Hospice staff is routinely tested every 14 days for Covid -19.  Leveraging over 24 years of hands on experience that I have as a registered nurse, our leadership has been diligent about anticipating the current state of affairs and vigilantly preparing. As our healthcare community prepares for a potential Coronavirus outbreak, my team at SilverStone Hospice, along with our sister company V-Care Home Health, is ready to assist you in the following ways:

PREVENTION & CARE TEAMS: Our clinical and support staff are trained in infection control and reporting systems specifically related to COVID-19. Our team begins each morning with daily field-staff health check-ins and have had no suspected cases of COVID-19. SilverStone Hospice has not been notified of any employee or family member with the virus. We are equipped with the N95 masks, disposable gowns, and gloves, as well as supplies to provide care for acute respiratory distress; this allows you to operate in your facility while eliminating risk to your patients.

OUR CLINICAL CARE TEAM: Including our transitional care volunteers, are always accessible to you even in the most high-risk patient cases. We are actively supporting assisted living facilities in spite of the generalized lock down to outside personnel. With our team, no patient suffers alone, no family struggles alone, no loved one dies alone.

PHYSICIAN SUPPORT: Our Board Certified House Call team of Doctors and Nurses are in place to maintain care of both stable and post-acute care patients. Our Doctors are also able to provide COVID-19 test kits for your patients, if they are under our care. Telemedicine is available for those with higher risk of contagion.

MORE ROOM FOR YOUR PATIENTS: When your facility needs more room to accommodate for treatment of acute patients, our social workers and case managers will collaborate with your team to provide a safe and comfortable transfer for non-critical residents into their personal home environment, or our partner network of Skilled Nursing Facilities, ALF and Group Homes to receive personalized care from our team. Over 50 rooms are immediately available where your patients will receive our Comprehensive Care Program that includes Home Health, Hospice, and Home Dialysis.
